Abstract

Introduction. Benign tumors and non-neoplastic lesions of bone tissue account for the majority of primary pathological bone lesions in children. Despite the benign nature of these lesions, there is a significant risk of pathological fractures. However, there are no generally accepted criteria for assessing the risk of pathological fracture in pediatric orthopedic practice, which complicates the choice of treatment strategy. In particular, an important issue is the standardization of indications for prophylactic osteosynthesis in such patients.

Aim. To assess the risk of pathological fracture using a modified scale and to substantiate the use of prophylactic osteosynthesis in the treatment of an 11-year-old patient with a cyst of the femoral neck.

Materials and methods. A retrospective analysis of the medical records of an 11-year-old patient with a cyst of the femoral neck was performed. The risk of pathological fracture was assessed using the modified scale described by Li et al. The indications for prophylactic osteosynthesis were substantiated based on the obtained data.

Results. The use of the modified scale allowed for an objective assessment of the characteristics of the bone lesion. The total score was 8, which corresponded to the indications for prophylactic osteosynthesis. Fixation was performed using an LCP plate and screws. During the 7-month follow-up period, positive clinical and radiological dynamics were observed, with no complications.

Conclusions. This clinical case demonstrates the applicability of the modified scale for assessing the risk of pathological fracture and determining indications for prophylactic osteosynthesis in children with benign bone lesions.
